General Overview: How to Apply

Applicants can now initiate a passport application from the comfort of their home. Please follow this two-step process strictly:

  1. Step 1: Federal Portal: Visit the Nigeria Immigration Service Portal to complete the application form, upload documents, and make your payment. You must print your Application Form, Payment Slip, and Appointment slip, all uploaded documents including certificate of identification from State laison Office/Local Government of origin or Attestation of citizenship from the Embassy
  2. Step 2: Embassy Appointment: After completing Step 1, applicants should complete the NIS application form, upload documents, make payment and book an appointment via the Embassy’s Appointment Booking Link.
Important: Your appointment on the NIS portal must take place before your appointment at the Embassy. Failure to bring your printed NIS appointment slip will result in the inability to process your biometrics.

Service Process

Passport Requirements for Minors (Under 18)

Application & Payment: Printed NIS application form and payment receipt.

Embassy Appointment: Printed booking confirmation from the Embassy portal.

Parental Consent: A letter of consent signed by both parents.

Identification: Data pages of both parents’ expired Nigerian passports for signature verification.

Birth Certificate: Original and translated/legalised copies for foreign births, or National Population Commission (NPC) certificate for births in Nigeria.

Marriage Certificate: Required if the mother’s surname differs from the father’s. Foreign certificates must be translated and legalised by the Ministry of Foreign Affairs of the issuing country.

Citizenship Proof: State/Local Government Certificate of Identification or an Attestation of Citizenship from the Embassy.

Photos: Uploaded ICAO standard passport photographs.
Requirements for Passport Reissue (Adults)

• Current Passport: You must present your existing/expired Nigerian passport.

• Application & Payment: Printed NIS application form, appointment slip and payment receipt.

• Embassy Appointment: Printed booking confirmation from the Embassy portal.

• Birth Documentation: National Population Commission(NPC) for individuals born after 1992.

• Born before 1992: High Court Affidavit for Declaration of Age (with photo and Commissioner for Oaths signature).

• Citizenship Proof: State/Local Government Certificate of Identification or an Attestation of Citizenship from the Embassy (With passport photo).

• Photos: Uploaded ICAO standard passport photographs.
Requirements for First-Time (Fresh) Passports
• Application & Payment: Printed NIS application form and payment receipt.

• Embassy Appointment: Printed booking confirmation from the Embassy portal.

• Birth Documentation: NPC Birth Certificate (post-1992) or High Court Affidavit (pre-1992).

• Citizenship Proof: State/Local Government Certificate of Identification or an Attestation of Citizenship from the Embassy.

• Photos: Uploaded ICAO standard passport photographs.
Change of Name Due to Marriage
• Formal Request: Signed application letter addressed to the Head of Mission stating the reason for the change.

• Evidence of Marriage: Duly legalised Marriage Certificate.

• Public Notice: :Newspaper publication for change of surname (must show newspaper name and date).

• Legal Affidavit: High Court Affidavit for change of surname.

• NIN Update: Updated National Identification Number (NIN) slip reflecting the new name.

• Supporting Docs: Husband and applicants data page, applicant's birth certificate, and State of Origin certificate.
Change of Name Due to Family Resolution or Religion

• Formal Request: Signed application letter to the Head of Mission. Use the (Official Downloadable Form Here). (I will provide it)

• Family Consent: Family resolution document signed by every member of the family.

• Public & Legal Notice: Newspaper publication and High Court Affidavit (showing Name of new paper and date of publication).

• Supporting Docs: NIN slip, applicant’s birth certificate, and data pages of other family members' passports, Local/State government identification, data page of all signatories to the resolution.
Correction of Date of Birth
• Formal Request: Signed letter to the Head of Mission explaining the error. Use the (Official Downloadable Form Here).

• Primary Evidence: Original NPC Birth Certificate or High Court Affidavit.

• Supporting Evidence: Secondary School Certificate (WAEC/NECO) and updated NIN slip, Local/State government identification.

• Legal Notice: High Court Affidavit for change of Date of Birth.
Requirements for Damaged Passports
• Formal Request: Signed letter to the Head of Mission explaining the error. Use the (Official Downloadable Form Here).

• Primary Evidence: Original NPC Birth Certificate or High Court Affidavit.

• Supporting Evidence: Secondary School Certificate (WAEC/NECO) and updated NIN slip, Local/State government identification, Passport data page where available.

• Legal Notice: High Court Affidavit for change of Date of Birth.

• Primary Evidence: Police report.

• Supporting Evidence: Secondary School Certificate (WAEC/NECO) and updated NIN slip.

• Legal Notice: Change of Date of Birth, Land/State government identification.

Critical Information for All Applicants

Mandatory Documents
A Birth Certificate/Affidavit and a State of Origin Certificate/Attestation are compulsory for every application, regardless of the passport type.

Payment Validity
All government payments expire after one year

Biometric Collection
You must bring original “breeder” documents for verification.

Passport Pickup
The Embassy will contact you once produced. You must bring your previous passport for cancellation at the time of pickup.

Integrity Policy
Do not engage in private financial transactions with any officers. All fees are paid via official channels
